Responsibilities
Triage level of services needed upon initial assessment for substance use disorders, mental health, and physical health needs.
Assess the status of clients related to their condition and environment and communicate the findings to the clinical team.
Carry out social evaluations and plan interventions based on evaluation findings.
Provide information and referral services for clients and potential clients regarding clinical and environmental needs.
Prepare and complete all necessary documentation and record-keeping.
Prepare individualized daily notes, showing accurate and personalized documentation regarding the client’s progress.
Participate in the development of the treatment plan of care and staffing meetings as required.
Participate in discharge planning for the level of care transitions and one-year follow-up of clients to monitor engagement in ongoing care and monitor for relapse potential.
Prepare lectures and facilitate groups that focus on family dynamics, parenting, and trauma.
Supervise the development of the care support team from admission to discharge from the HOPE program (12 months); including the development of social support in the family, community members, and referral sources.
Demonstrate knowledge and competency related to confidentiality and ethical standards of the social worker profession.
Work within a team setting- works effectively with a treatment team format, knows how to work with supervision.
Work in collaboration with stakeholders including funding sources to meet expectations.
Maintain knowledge of and adheres to The Walker Center’s policies and procedures.
Monitor client’s ongoing recovery following residential services and provide outcome data tracking and reporting.

Qualifications
We are looking for both a Licensed Master Social Worker (LMSW) (Licensed Clinical Social Worker (LCSW) preferred), as well as a Bachelor’s Level Social Worker. You must have either a bachelor’s or master’s to apply.
Two years of experience with chemically-dependent individuals preferred.
Anyone who is in recovery from a substance use disorder and identifies as a person in recovery must have a minimum of five years of sobriety from substances of abuse.
Must pass pre-hire requirements including a criminal background check, UA, and TB test.
